Trick movements

Thursday, May 29th, 2008

I am writing this post after having met one of my patient who is recently been operated for malunion of Ulna in forearm. Patient had complex both bone fractures in the past and was operated to fix them at multiple location. But when the rehabilitation phase started, the problems were more apparent, patient lost most of supination, but with some pronation.

This is is tricky because when you lose supination your upper limb cannont compensate because your body comes in way and prevent you to do all activities that require supination like doing clock wise motion of your hand(like putting a screws) and so on. If the loss of supination in forearm is acompinied by loss of flexion and extension at wrist, you have more disability. On the other hand if you loose your pronation, you can easily compensate by doning trick movements like abducting your shoulder instead of moving at forearm. People can cope really well folllowing loss of pronation compared to those people who lost supination.

Also do you know what muscle is strong supinator - it is famour Biceps. Biceps is famous in lay people mind as bulk arm muscle doing flexion. But it is more important as supinator, than as flexor of the elbow.

Leadership and essence of it (in context of US democratic primaries)

Tuesday, May 27th, 2008

One of the signs of leader is being strong, articulate and well prepared to any problems that they may face. Old proverb says “when going gets tough, tough gets going”. That is main essence of a leader. If people don’t have characteristics of leader but managed get that position based on other reasons like family position, money or similar things, they can’t acts as one. Their work tends to be chaotic, dependent on advisers and get desperate in tough situations. This comparison can be easily drawn between Barack Obama and Hillary Clinton.

Barack Obama came through lots of challenges all through his life. He was born to a black father and white mother. He was raised by single parent, gone to school with scholarships worked hard all the way to Harvard law school. He then went to Chicago to do community service, worked all the way to state senate, then to national senate and now he is pushing himself to become next president of America. Raise of Obama highlights traits one need to have to be a leader. His ability to manage campaign effectively, his approach to people, his novel way to get donations, his stand on his principle all show how effective he can be as a leader.

Hillary Clinton on the other hand was born with silver spoon, was wife of former president. Her career mostly was a cake walk. She became New York senator without significant fight. She was established candidate at the beginning. She chose team not based on talent but on past links. Her team did not grasp rules of nomination process; she was not prepared to run even for primaries. Her principles kept changing as she need, she started lying and finally she is accusing Obama campaign for her failures. In nut shell when going gets tough, she got desperate, chaotic, demoralised and lost her principles.

Proposal to change hepatitis blood donation rules in United States of America

Wednesday, May 21st, 2008

This is a new move by FDA to relax some rules regarding blood donation who were tested positive for hepatitis B. They suggest that older test (1980’s and 1990’s) to determine Hepatitis B resulted false positive test in some patients which prevented eligible donors from donating blood.According to new proposal people who were tested positive in the past will be screened again to check their Hepatitis and HIV status, and if they are found negative on thorough examination will again be allowed to donate blood.

How ever if those found positive again on repeated testing, they will not be allowed to donate blood. Also those who suffered hepatitis in past will not be allowed to donate blood.

Breast feeding and Breast cancer - different association

Sunday, May 11th, 2008

We all know that breast feeding by mother reduces incidence of breast cancer in her. But new reserach suggest that women who were breast fed when they were infants had low incidence of breast cancer later in their life

Adult women who were breast-fed as infants may have a lower risk of developing breast cancer than those who were not breast-fed, unless they were first-born, study findings suggest.

“As a general group, women who reported they had been breast-fed in infancy had a 17 percent decrease in breast cancer risk,” Hazel B. Nichols, who was involved in the study, told Reuters Health.

“However, we did not observe this reduction when we looked specifically among first-born women,” said Nichols, of the University of Wisconsin, in Madison. link
